What Doctors Need, Country by Country

Doctors

United States

OET Medicine is the English requirement for the ECFMG Certification Pathways, the route to US residency for international medical graduates.

Planning for the 2027 Match? Take OET Medicine by the last test date in December 2026. Results usually arrive in about 10 business days, and you need your MyIntealth ID to release them to ECFMG.

Doctors

United Kingdom

The General Medical Council (GMC) accepts OET Medicine with a grade B in each sub-test.

Good news: doctors registered with the GMC can use that registration to meet the English rule for the Health and Care Worker visa.

Doctors

Ireland

The Medical Council of Ireland accepts OET Medicine for registration. Check the current grades on the Council’s website before you book.

Doctors

Australia and New Zealand

The Medical Board of Australia and the Medical Council of New Zealand accept OET Medicine, usually with a grade B in each sub-test.

For Australian visas, OET must be taken at a test centre. Check whether OET@Home is accepted for your registration before you book.

Doctors

Canada and beyond

Several Canadian medical regulators and health authorities in other countries, including the United Arab Emirates, recognize OET. Each one sets its own rules, so check yours.

OET does not count for Canadian Express Entry.

Requirements change, so always confirm with your regulator or ECFMG before you book. Last checked October 2026.

The OET Test, Explained

Four sub-tests, about three hours in total. Listening and Reading are the same for every profession. Writing and Speaking are written for doctors.

About 40 minutes

Listening

Three parts: consultation extracts, short workplace recordings, and longer presentations or interviews on healthcare topics.

60 minutes

Reading

Three parts: a fast search across short texts, workplace documents such as guidelines, and longer healthcare articles.

45 minutes

Writing

One professional letter based on case notes, with 5 minutes to read and 40 minutes to write.

Doctors write a referral, discharge or transfer letter, usually to another doctor, based on a patient’s case notes.

About 20 minutes

Speaking

Two role plays with an interlocutor, each with 3 minutes to prepare and about 5 minutes to speak.

You play the doctor. Typical role plays include taking a history, explaining a diagnosis or treatment, and reassuring an anxious patient.

How OET Is Scored

Each sub-test is scored from 0 to 500 and given a grade. Most regulators ask for a grade B in every sub-test.

A450 to 500
B350 to 440, the usual target
C+300 to 340
C200 to 290

Writing is marked on six criteria

PurposeContentConciseness and clarityGenre and styleOrganisation and layoutLanguage

Our platform gives you feedback on all six after every letter, so you know exactly which one is holding your score down.

Speaking is marked on two sets of skills

IntelligibilityFluencyAppropriate languageGrammar and expressionRelationship buildingThe patient’s perspectiveStructureGathering informationGiving information

Clear English is only half of it. You are also marked on how well you communicate with the patient, and that is where accent and communication coaching makes the difference.

Where Doctors Lose Points

1

Writing the whole case

The reader needs only what matters for the next step of care. Including every note costs marks for Content and Conciseness.

2

Medical jargon with patients

Terms that are perfect with colleagues confuse patients. Plain, kind explanations score higher in Speaking.

3

Running the consultation, not sharing it

Role plays reward checking understanding, inviting questions and responding to feelings, not just giving information.

4

Pronunciation that slows the listener

Stress and intonation patterns from your first language can make clear medical English hard to follow. Accent coaching fixes this directly.
